UBLES Enrollment Advisory for SY 2024-2025

The University of Baguio Laboratory Elementary School (UBLES) now welcomes applications for the School Year 2024-2025. We are looking forward to seeing you next school year, young UBians! 

PREPARE THE FOLLOWING REQUIREMENTS:

1. Valid ID of Guardian 
2. 2×2 Picture of Pupil
3. Report Card/SF9
4. PSA Birth Certificate
5. Form 137
6. Certificate of Kindergarten Completion – For Incoming Grade 1 Students
7. Baptismal Certificates (For incoming Grade 4 Catholic Students Only)

FOR NEW PUPILS/TRANSFEREES AND RETURNING STUDENTS: 

1. The student / parent / guardian registers to https://portal.ubaguio.edu. 
2. Click “Apply as a New Student”, complete all the needed information and submit the registration. You will receive an email containing your APPLICANT ID NUMBER that you will be used to pay the Reservation fee
3. You will receive a confirmation email with the link for the submission of scanned copy of the admission requirements and your APPLICANT ID NUMBER that you will be used to pay the Reservation fee
4. Pay the Reservation Fee of Php 1,000.00 to any UB accredited Banks.
   *Admission Processing Fee is NON-REFUNDABLE but will be credited to the tuition fee once enrolled.

5. Prepare the following documents which will be uploaded in the google form:
   a. Valid ID of the Parent / Guardian (scanned).
   b. Student’s Report Card (SY 2022-2023) with LRN number (Grades 1-6 only) given by the adviser from his/her previous school.
   c. Student’s Birth Certificate (scanned)
   d. Scanned Baptismal Certificate (for Grade 4 Catholic students only)
   e. Registration Fee Proof of Payment (Php 1,000.00) to be paid in UB’s Accredited banks
6. After accomplishing the Pre-Enrollment Form, the student / parent /guardian will pass the following documents to the enrolment venue:
   a. Original Report Card/ SF9
   b. Photocopy of Birth Certificate (3 copies)
   c. Photocopy of Baptismal Certificate (for Grade 4 Catholic students only)
   **PTA Fee to be paid at the enrolment venue
7. Proceed to the enrolment venue to fill out the Individual Inventory Form and have a short interview with the parent.
8. Pay the down payment in any of the accredited banks, online bills payment or at the UB Cashier.              
9. Wait for a text message or e-mail from his/ her Class Adviser.

FOR OLD STUDENTS 

1. The student / parent /guardian registers in the Enrollment form in this link  https://forms.gle/kYprvLf3wM7Fdt1JA
2. After accomplishing the form, wait for a confirmation e-mail from UBLES that the student have already been enrolled.
   **Registration for students with unpaid school fees from previous school year is locked. The balance should be fully paid before the registration of subjects.
3. Fill out the UBLES-Center for Counseling and Student Development Cumulative Record Form via google form which will be sent together with the confirmation email.
4. Pay the down payment in any of the accredited banks, online bills payment or at the UB Cashier.
   ** PTA Fee to be paid at enrolment venue.
5. Wait for a text message or e-mail from his/ her Class Adviser.
